Replacement Plates and/or Stickers

Replacement Plate

Plates are replaced on vehicles when one or both plates have been lost or plates are defective.

Complete a vehicle application form or provide written request for a replacement which includes:

plate number of vehicle (as displayed on the vehicle registration document) signature of registered owner
If both plates are missing, you must complete lost plate affidavit available at any Motor Registration office in your area.
If one plate is missing applicant must return remaining plate with the application to any Motor Registration office in your area;
If plates are defective applicant must return both plates to a Motor Registration Office. Defective plates are replaced at no charge, with supervisor authorization.

Defective plate is replaced at no charge, with supervisor authorization

You must complete a Lost Sticker Affidavit if sticker is lost or destroyed available at any Motor Registration office in your area.
Affidavit must be completed by registered owner.
If sticker is defective, applicant must return sticker and it will be replaced at no charge with supervisor authorization.
