Driver Licence Replacement

If your drivers licence is lost,stolen, or damaged you can apply in person or in writing by mail or fax, to the nearest Motor Registration Division office in your area for a replacement. If you know your driver's licence number, please provide it with your request. If you do not, the following information is required:

name and address, and
date of birth.

There is a $15.00 fee to replace your drivers licence.
