A registration number may be issued to a person or firm seeking to install or make alterations to an elevating device or an amusement ride.

Requirements

To be prepared and submitted in duplicate
Completed copies of the specification sheet; For Elevating Devices and Amusement Rides
Duplicate drawings of the elevating device and hoistway design. Engineering drawings must be signed and sealed by a professional engineer
Two copies of design drawings bearing the signature and professional seal of the person responsible for the design of an amusement ride
